The Fasting Blood Sugar (FBS) test measures blood glucose concentration after at least 10–12 hours of fasting — when the digestive system has cleared and glucose reflects the body's baseline regulation without dietary influence. FBS is the primary and most reliable test for diagnosing Type 2 diabetes, prediabetes (impaired fasting glucose), and monitoring blood sugar control in diabetic patients. It is one of the most important and frequently ordered tests in preventive health and chronic disease management. 10–12 hours fasting required.
Key benefits of this package include:
What Does the Glucose Fasting Test (FBS) Measure?
| Level / Result | Possible Interpretation |
|---|---|
| 70–99 mg/dL (Normal) | Normal fasting glucose — no evidence of diabetes or prediabetes |
| 100–125 mg/dL (Prediabetes — IFG) | Impaired Fasting Glucose — prediabetes; lifestyle intervention can reverse progression to diabetes |
| ≥ 126 mg/dL (Diabetes) | Consistent with diabetes — requires confirmation with repeat fasting glucose or HbA1c on a separate day |
| < 70 mg/dL (Hypoglycaemia) | Low blood glucose — evaluate symptoms; causes include excess insulin, prolonged fasting, alcohol, Addison's disease, insulinoma |
| FBS 80–130 mg/dL in a treated diabetic | Within the ADA target range for most diabetic patients on therapy — adequate glycaemic control |
| FBS > 180 mg/dL in a treated diabetic | Above target — medication adjustment, dietary review, or investigation of contributing factors needed |
Important Note: A single FBS ≥ 126 mg/dL is not sufficient to diagnose diabetes — confirmation requires a repeat FBS ≥ 126 mg/dL on a separate day, OR HbA1c ≥ 6.5%, OR OGTT 2-hour glucose ≥ 200 mg/dL, OR random glucose ≥ 200 mg/dL with classic symptoms. In asymptomatic patients, two abnormal test results from two different tests or two separate occasions are required. All results must be reviewed by a physician.
Prediabetes — The Most Important Finding in Preventive Medicine: Prediabetes (FBS 100–125 mg/dL) affects an estimated 100 million Indians — making it the largest modifiable risk pool for Type 2 diabetes. At this stage, progression to diabetes is not inevitable: structured lifestyle intervention (5–7% weight loss, 150 minutes of moderate exercise per week, and reduced refined carbohydrate intake) reduces progression by 58% — more effective than metformin alone. A fasting glucose of 110 mg/dL identified through routine screening — treated with lifestyle changes — can prevent decades of diabetes, its complications, and significantly reduce cardiovascular risk. This is the central argument for making FBS a routine annual test for all adults above 30.
