The Random Blood Sugar (RBS) test measures blood glucose at any time of day, regardless of the last meal. It is the most convenient initial glucose test — requiring no fasting or meal timing — and is used when a quick glucose assessment is needed, particularly in patients presenting with classic symptoms of diabetes (excessive thirst, frequent urination, unexplained weight loss) or hypoglycaemia. A random glucose ≥ 200 mg/dL with typical symptoms is sufficient to diagnose diabetes without requiring fasting confirmation. No fasting required.
Key benefits of this package include:
What Does the Glucose — Random Test (RBS) Measure?
| Level / Result | Possible Interpretation |
|---|---|
| 70–139 mg/dL (Normal range, random) | Blood glucose within expected range at this time of day |
| 140–199 mg/dL (Elevated — random) | Elevated glucose — may indicate prediabetes or diabetes, especially if post-meal; FBS and HbA1c recommended for proper assessment |
| ≥ 200 mg/dL with symptoms (Diabetes) | Diabetes diagnosis confirmed with classic symptoms — confirm with FBS or HbA1c; initiate management |
| ≥ 200 mg/dL without symptoms | Significant hyperglycaemia — requires confirmation with FBS ≥ 126 mg/dL or HbA1c ≥ 6.5% on a separate day |
| < 70 mg/dL (Hypoglycaemia) | Low blood glucose — assess symptoms; manage hypoglycaemia; investigate cause |
Important Note: Random blood glucose interpretation depends heavily on timing relative to meals. A glucose of 180 mg/dL taken 90 minutes after a carbohydrate-rich meal in a non-diabetic is within the expected postprandial range. The same value fasting is significantly abnormal. Without knowing the meal timing, random glucose is a screening tool — not a diagnostic one. For definitive diabetes screening, FBS (Test 81) and HbA1c are preferred. All results must be reviewed by a physician.
When Is RBS the Right Test? Random blood glucose is most appropriate when: (1) the patient has classic symptoms of hyperglycaemia and a rapid glucose result is clinically needed — RBS ≥ 200 mg/dL with symptoms diagnoses diabetes without waiting; (2) hypoglycaemia symptoms require immediate glucose confirmation; (3) monitoring glucose in hospitalised or critically ill patients where meal timing is variable; (4) community health screening where standardised fasting is not feasible. For routine preventive screening and definitive diabetes diagnosis in asymptomatic individuals, FBS and HbA1c are superior.
