The Luteinising Hormone (LH) test measures the level of LH — a pituitary hormone that triggers ovulation in women and stimulates testosterone production in men — in the blood. LH works in tandem with FSH as one of the two gonadotropins regulating reproductive function. In women, the mid-cycle LH surge directly triggers egg release (ovulation) — making LH a key marker of ovulatory function and fertility. In men, LH stimulates testicular Leydig cells to produce testosterone. LH is an essential component of both female and male fertility workups. No fasting required.
Key benefits of this package include:
What Does the Luteinising Hormone (LH) Test Measure?
| Level / Result | Possible Interpretation |
|---|---|
| Women — Follicular phase LH: 2–15 IU/L (Normal) | Normal early follicular LH — ovarian axis functioning |
| Women — Mid-cycle LH surge: 22–105 IU/L | Normal ovulatory LH surge — ovulation imminent within 36–42 hours |
| Women — LH:FSH ratio > 2:1 (Early follicular phase) | Elevated LH:FSH ratio — consistent with PCOS pattern; assess alongside testosterone, AMH, and pelvic ultrasound |
| Women — LH > 40 IU/L + FSH > 40 IU/L (persistent) | Ovarian failure — menopause or premature ovarian insufficiency (POI) if age < 40 |
| Men — LH low (< 1.7 IU/L) + Low testosterone | Secondary hypogonadism — pituitary or hypothalamic failure reducing LH drive; MRI pituitary evaluation |
| Men — LH elevated + Low testosterone | Primary testicular failure — testes are not responding to normal LH stimulation; assess with FSH, karyotype |
| Children — LH elevated with early puberty signs | Precocious puberty — central (gonadotropin-dependent) type; paediatric endocrinology evaluation |
Important Note: LH must always be interpreted alongside FSH, estradiol (in women), testosterone (in men), and the clinical picture. In women, the menstrual cycle day of blood collection is critical — mid-cycle LH surge values are normal, not a sign of disease. Single LH measurements are less informative than paired FSH + LH ratios. All results must be reviewed by a reproductive endocrinologist, gynaecologist, or urologist.
LH in Fertility Treatment — Monitoring Ovarian Stimulation: In IVF and ovulation induction protocols, serial LH monitoring (alongside estradiol and follicular ultrasound) is used to track follicular development and identify the optimal moment for HCG trigger injection (which mimics the LH surge). Premature LH surges — occurring before adequate follicular maturity — can compromise IVF cycle outcomes. GnRH agonist and antagonist protocols suppress premature LH surges, and monitoring is essential to ensure the surge is properly controlled and timed.
