The Typhidot IgM test is a rapid immunochromatographic assay that detects IgM antibodies specific to Salmonella typhi outer membrane protein (OMP) in blood — providing a fast, sensitive, and specific diagnosis of typhoid fever (enteric fever). Unlike the traditional Widal test (which has significant false-positive rates), Typhidot directly detects Salmonella-specific antibodies with greater specificity. IgM antibodies appear early in infection (Day 4–7 of fever), making Typhidot a valuable rapid diagnostic tool in the acute phase of illness. No fasting required.
Key benefits of this package include:
What Does the Typhidot IgM Test Measure?
| Level / Result | Possible Interpretation |
|---|---|
| Negative | No IgM antibodies to S. typhi detected — typhoid less likely in acute phase; repeat if clinically suspected; blood culture if available |
| IgM Positive (Typhidot IgM positive) | Active typhoid fever — S. typhi-specific IgM detected; initiate antibiotic therapy (azithromycin or ceftriaxone); blood culture for confirmation and sensitivity |
| IgM Positive + Clinical fever > Day 4 | High probability of active typhoid — treat empirically pending culture; avoid fluoroquinolones if resistance is locally prevalent |
| IgM Positive in vaccinated individual | May reflect vaccination response or true infection — correlate with fever duration, clinical picture, and blood culture |
Important Note: Typhidot IgM has sensitivity of approximately 70–80% and specificity of 80–90% for acute typhoid. A negative result does not exclude early typhoid (first 3–4 days before IgM appears). Blood culture remains the gold standard for definitive diagnosis and antibiotic sensitivity testing. Prior typhoid vaccination may cause false-positive results. All results must be reviewed by a physician — typhoid requires appropriate antibiotic therapy and clinical monitoring for complications.
Typhidot vs. Widal Test — Why Typhidot Is Preferred: The traditional Widal test (agglutination test for anti-Salmonella antibodies) has been widely used in India for decades but is significantly limited by: high false-positive rates from cross-reactions with other Salmonella species, previous infection, or vaccination; high background positivity in typhoid-endemic populations making interpretation difficult; and the need for paired sera (taken 2 weeks apart) for reliable results. Typhidot detects OMP-specific IgM antibodies with greater specificity and requires only a single sample. Most modern clinical guidelines now prefer Typhidot or blood culture over the Widal test for typhoid diagnosis.
